I'm considering an Ohlins rear shock S46DRI (I think that is the right number - it is the shock without external adjustment devices).

I want the shock as I'm 230 lbs and I ride with my wife / full bags and top case some of the time.

I'm wondering if anyone has experience with manually adjusting this model of shock on the FJR and the accessibility of getting to it to adjust when all mounted.

 
That's the YA053. I have the YA036. It's pretty much the same shock and it isn't hard to adjust. It comes with a spanner. If you ride fully loaded most of the time you won't need to adjust it much. If you have to adjust often, then the YA707 is the way to go but, it's twice the price.

 
I ordered my Ohlins via GP Suspension at group buy time. After a chat with them mine arrived almost perfectly set from them. I think they were a couple of clicks off on damping and that was it. Recommend them highly.
