Torque rod bolt failure

I would surmise not tightened also. That bolt is pretty stout and really gets little tension that would cause failure. Now rust etc could be a factor and that would be seen at time of change to replace it.

Glad your service department is helping, they are golden (In repairing it no charge, not forgetting to tighten it)! Keep them.
